This document contains programming instructions for the Projects for Assistance in Transition from Homelessness (PATH) Annual Report and is the result of HUD and the Substance Abuse and Mental Health Services Administration (SAMHSA) working to ensure that the HUD HMIS Data Standards and the PATH Annual Report form fully align.
