To register for a new user training, please complete a ServicePoint End User License Request form, which can be downloaded at https://www.icalliances.org/hmis-forms-and-documents-south-carolina and return to Brandy at hmis@one80place.org. Completed forms must be received by close of business the Friday before the training date.
