Thank you for your interest in joining our HMIS network!  Here are the steps to join:

  • NEW to Atlanta COC? GET INVOLVED!

  • NEW ORGANIZATIONS

    • For New Agency - Click Link on the form for the HMIS Access Request Form.

      • Someone from their team will reach out to connect with you to set up some time to connect and discuss your organization and HMIS

      • If approved, you will be sent a formal HMIS Agency Partnership Agreement. In this form, be sure to:

        • Review HMIS Policy and Procedures, etc - appendices to agreement

        • Review Release of Information and incorporate into Agency Privacy Planning (post sign at intake, offer ROI for all intakes, have copy of privacy policy available at request)

        • Note, in the APA and Project Requests, a physical Atlanta address designates Atlanta as the CoC. Some agencies and projects may cross CoC boundaries designated by HUD.

          • Ask questions!

            DV COMPARABLE DATABASE (Use for agencies designated as Victim Service Providers (VSP’s) or that receive funding that prohibits use of HMIS (Does the agency receive funds from the Family Violence Prevention and Services Act (FVPSA), Office for Victims of Crime (OVC), or Office on Violence Against Women (OVW) and use those funds agency-wide or for agency administrative purposes?)

            *Funding Note: Please review the Comparable Database Decision Tree to determine if your agency should enter data into a comparable database. If yes, please let the Partners for Home team aware of this so we can get you set up with the DV Comparable Database administrator.

  • PROJECTS (Typically, you will create one project for each/ new Program or Grant)

    • Project Set Up forms can only be submitted by the HMIS Agency Administrators (or Executive Director)

      • *Note: For new orgs onboarding, guidance will be provided once your organization has been approved for participation in HMIS.

    • The link to submit a new project is available in ClientTrack & only visible to approved HMIS Agency Administrators.

    • Please prepare to identify the following, at the time of completing the form:

      • Project Type

      • Grant funding information*

      • Project address information (Atlanta CoC should be selected based on physical address and grant funding)

      • Housing information (beds, units, population served)

NEW END USERS

  • *UPDATE: End User requests are now initiated by the HMIS Agency Administrators (AA) or Executive Director.

    • Once the AA submit the request form on behalf of the End User, the CoC Administrator will review for approval.  If approved, the End User will be sent the End User Participation Agreement (EUPA) to sign. Once signed, the CoC Admin will then set up training for the End User who will then receive their TalentLMS credentials to complete set of training courses.  Once the TalentLMS courses are complete, GAHMIS will set up the ClientTrack account.

      • If the request is denied, you will receive notification for why the form was denied.                                                            

    • The link to start a request is available in ClientTrack & only visible to approved HMIS Agency Administrators.

    • *NOTE: Users will only have 15 days to complete the trainings before being marked as inactive. Please start courses once you receive login credentials.

    READ ONLY USERS, ACTIVE USERS SWITCHING ORGS, or USERS REQUESTING ACCESS TO AN ADDITIONAL ORGANIZATION

    • If the request is for a READ ONLY user, the process is the same but will NOT have to complete the TalentLMS training.

    • If the request is for an End User who is SWITCHING ORGS/ NEEDING ADDITIONAL ORG ACCESS, the process is the same.  Training requirements will depend on when the end users was last in the system.
